Pregnancy in Functional and
Integrative Medicine

A pregnant woman who swaps table salt for pink salt and cuts out ultra-processed foods may, without noticing, cut out her main source of iodine, precisely when her need rises to 220 mcg a day and the baby's brain depends on the mother's thyroid hormone. At the other extreme, a single drop of 5% Lugol's solution contains almost 30 times that need. This e-book shows where the "healthy" pregnancy goes wrong and delivers the fine-tuning: active forms, doses by trimester, 22 compounded formulas and Dr. Estela's guidance.

  • Advanced supplementation: the right active form, dose and timing
  • 22 compounded formulas, from preconception to breastfeeding
  • Pregnancy guidance: food, a toxin-free home and sleep

What almost no one tells you
about supplementation in pregnancy.

Between the pharmacy multivitamin and tips from social media, pregnancy is usually managed by the bare minimum. These are six points the e-book takes apart, each with the studies that support it and the module where the topic is covered in detail.

  • 01Modules 5 and 20

"Swapping table salt for pink salt is always healthier"

In pregnancy, it may not be. In Brazil, iodized salt is the main source of iodine, and many specialty and imported salts are not iodized. A woman who switches salt, cuts out ultra-processed foods and eats little fish and dairy can fall well below 220 mcg of iodine a day without noticing. In São Paulo, 57% of the pregnant women assessed were below the adequate level.

  • 02Module 5

"Lugol's iodine raises the baby's IQ"

Iodine protects the baby's brain, but in micrograms. One drop of 5% Lugol's solution contains about 6,300 mcg of iodine, almost 6 times the tolerable upper intake level, and three babies with congenital hypothyroidism have been described whose mothers took 12.5 mg a day during pregnancy or breastfeeding, the equivalent of two drops. The e-book provides the formula for Lugol's diluted to 50 mcg per drop.

  • 03Modules 2 and 4

"Folic acid starts when the test comes back positive"

The neural tube closes between the 21st and 28th day after conception, when many women do not even know they are pregnant yet. And stores take time: after 24 weeks of supplementation, red blood cell folate had still not reached a plateau. That is why the e-book's protocol starts three to six months before, together with vitamin B12.

  • 04Modules 6 and 7

"The prenatal multivitamin already has everything"

Almost none provides enough choline and DHA. Choline is required in amounts hundreds of times greater than folate, and in the United States only 8.5% of pregnant women reach the recommendation. In the Cornell trial, the babies of mothers who received 930 mg a day had faster reaction times, a marker of processing speed.

  • 05Modules 9 and 14

"Iron in pregnancy: the more, the better"

Iron prevents anemia, but in excess it raises hemoglobin above the ideal. In a cohort of 278 children, those born with the lowest cord ferritin performed worse at age 5, and those with the highest were more likely to have a low IQ. In the e-book, the dose comes from ferritin, and intravenous iron comes with indications, dose calculation and drip rate.

  • 06Module 20

"One small coffee a day does no harm"

Caffeine crosses the placenta, and the fetus lacks the enzymes to metabolize it. Traditional guidelines accept up to 200 mg a day, but a review of 48 studies concluded that pregnant women and women planning to conceive should avoid it, and in two American cohorts even low intakes were associated with shorter children in childhood.

The e-book does not replace prenatal care or a consultation. All supplementation is individualized, prescribed by a licensed professional and reported to the obstetrician, and what is the author's clinical practice is marked [PA], kept apart from what is backed by studies.

From preconception to postpartum,
with the evidence for each step.

The functional view of pregnancy

Conventional medicine asks: "is there a deficiency or a disease?". Functional medicine asks: "does this body have everything it needs to function optimally, and what is getting in the way?". In pregnancy, this means moving beyond the minimum needed to avoid disease (the laboratory reference value) and seeking the optimum to build a healthy baby and preserve the mother's health.

Advanced supplementation: the form, the dose and the timing of each nutrient

It is not "a prenatal multivitamin": it means starting from lab tests, choosing the most bioavailable form and adjusting the dose throughout the trimesters.

  • Active folate (5-MTHF) and methylcobalamin, the MTHFR variant and homocysteine as a gauge of methylation
  • Iodine and IQ: what each study showed, unrefined salt and the dose of 150 mcg a day
  • Choline and DHA: the Cornell study and the ADORE trial, in which high-dose DHA reduced early preterm birth, especially in pregnant women with low DHA
  • Vitamin D with a target of 40 to 60 ng/mL, iron guided by ferritin and the author's functional lab panel
  • Nutraceuticals with clinical trials in pregnancy: myo-inositol, coenzyme Q10, L-arginine, lactoferrin and strain-specific probiotics

Part 3 · modules 11 to 19

22 compounded formulas and supplementation by goal

Each formula with composition, dosage and indication, and each goal with the approach and the warning signs.

  • Pregnancy Lugol's diluted to 50 mcg per drop, to deliver micrograms and not milligrams
  • Formulas for nausea, gestational diabetes, preeclampsia, anemia, thyroid, allergies, constipation and cramps
  • The supplementation calendar by trimester and the daily schedule, from breakfast to bedtime
  • Intravenous iron when the oral route is not enough: indications, contraindications, Ganzoni formula, dilution and drip rate
